No, this is not an insurance plan. The Jolley Orthodontics orthodontic discount plan for teachers offers orthodontic services at a discounted rate at the time of the treatment agreement.
Does this include my dependents?
Yes, your spouse and children 18 and under are also eligible for the discount.
How do I use this plan?
Call Jolley Orthodontics at (817) 491-1600 to set up your FREE consultation. At the time of your visit, provide proof of employment to the Treatment Coordinator to receive your discounted fee.
What do I need to participate?
You will need proof of employment, proper identification, and proof of dependents.
If I go for a consultation, am I required to start treatment?
No. Your free consultation is used solely to determine your treatment plan, costs, and the estimated time of treatment.
